W-2 Information

General information about the W-2 form and associated requests.

IRS IP PIN Program

An IP PIN is a six-digit number assigned by the IRS to eligible taxpayers to help prevent the misuse of their Social Security numbers on fraudulent federal income tax returns. The IP PIN is not the 5-digit e-File PIN, and cannot be used for that purpose. More information is available here.

Electronic Consent

Active UF employees are able to access their IRS W-2 forms electronically through the myUFL system. In order to obtain on-line access, employees must first consent to receive the electronic version of the form.

Employee Year-End Earnings Statement (EYES)

The Employee Year-End Earnings Statement (EYES) assists employees in comparing the amounts on their W-2 forms to the earnings, deductions, taxes and net pay on their last biweekly earnings statements for the year.  The EYES also includes other nontaxable items as well as the University’s contributions on behalf of the employees and a detailed explanation of your taxable gross income.  EYES statements may be accessed through the myUFL system by navigating to My Self Service > Payroll and Compensation > UF Employee Year End Statement.

IRS 1042-S Forms

IRS 1042-S forms will be mailed on or before March 15.  Please note that if you are a nonresident alien who receives a 1042-S form, you will not be able to access the EYES mentioned above.  To obtain a corrected EYES, please e-mail fa-tax-forms@admin.ufl.edu.

W-4 Exempt Withholding Status and IRS Withholding Calculator

We are not permitted to give tax advice regarding your tax status or the number of allowances to claim when completing your Form W-4.  Please refer to the IRS Withholding Calculator or your tax preparer for guidance.

A Federal Form W-4 update must be received by February 15 of each year to ensure the continuation of your exempt tax withholding status for the calendar year. You may update your Form W-4 electronically through myUFL by navigating to My Self Service > Payroll and Compensation > W-4 Tax Information. If your Form W-4 is not updated by February 15, your tax withholding status will be changed to “single” with zero allowances. This change is required by IRS regulations and will remain in place until you submit an updated Form W-4. Tax refunds will not be made for failure to update your Form W-4 in a timely manner.

If you have questions, please call University Payroll & Tax Services at (352) 392-1231.
