Expenditures Involving Personally Identifiable Health Information (PHI)
Directive Statement
Electronically submitted invoices from suppliers must NOT include PHI. Health Science Center (HSC) departments are permitted to receive and retain invoicing at the Department level, a Transaction Authorization Form, and the HSC-PHI Cover Sheet in place of the supplier invoice detail.
Reason for Directive
To ensure the University is in compliance with all policies, laws, and regulations regarding PHI.
Who must comply?
All University Departments.
