Report of Survey
The Report of Survey form is used to request relief of accountability of property when disposing of property through Destroyed, Damaged, Worn-out, Obsolete, Operative, Cannibalization, Trade-ins, Transfers off UF Campus, and other dispositions.

Report of Survey - Lost/Stolen
The Report of Survey - Lost/Stolen form is used to request relief of accountability of property when disposing of property that has been stolen or lost.

Report of Transfer
The Report of Transfer form is used to initiate a transfer between UF Departments and Accountable Officers.

Refrigerant Removal Certification
This form is used when disposing of any item that contains refrigerants.

Off-Campus Property Certification
The Certification letter is used to account for property that is not on the UF Campus and is unavailable for inventory scanning purposes and should be completed whenever property first leaves the campus.

Property Update Document
The Property Update Document form is used to provide for a more efficient means of receiving a decal for newly purchased equipment.

Property Custodian Signature Authorization
This form is used to grant authority to the required Property Custodian for each departmental account number.

Authorization for Mailouts to Contact Personnel
This form is used to direct Property mailouts that do not require the Accountable Officer's signature to departmental contacts.
